Unfortunately, CityTours theme has not such feature.

You can not add/remove fields in booking form.

I would like to add a categories on the booking form. Now I have 2 categories (Adults and Students), the third category (children) is not on the first step of the booking form. 

I attached the screenshot (named "booking 1") that it's the first step of the booking form, in which there are just 2 categories and the secondo screenshot (named "booking 2") with the third category on "add options services". 

I would like to have all the 3 categories (adults, students, children) on the first page of the booking form.

There is anyone who can help me?
